• Describe the five domains and conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, function and quality-of-life outcomes and risks
• Determine, for the populations with which you work, which national objectives related to the social determinants of health highlight how personal social, economic and environmental factors can impact health.
• Promote Healthy People 2030 in your work

Public and community health reaches beyond medicine or health. Improving the health of all Americans requires collaboration of a health workforce, health and non-health sectors, and stakeholders guided by a strategic agenda. The agenda should include a vision, mission, goals and objectives with targets, leading health indicators; a plan of action with evidence-based interventions; and implementation and evaluation tools and resources.
