Thank you for signing up to take the NCHEC approved CHES CECH self-study self-paced and online course on Tracking (monitoring and evaluating programs for) Healthy People 2020 and for supporting the Community Too Box.

Feel free to send email to clarify course requirements and tell me about problems you may have while doing the course. Please remember that you will need to take a 10 to 15 question test and pass with a score of 70% or greater. Also, NCHEC requires each participant to complete an evaluation (included at the end of the test form). Both parts must be completed to get the 9.0 Category 1 (includes 2.0 advanced) continuing education credit hours .

15-question multiple choice and short answer post-course assessment (required, take test online, download test form and be sure to save form in directory or folder on your computer, take test, click submit button if you use Microsoft Outlook for email or email saved pdf form/test as an attachment to ce@healthedpartners.org.
